This form is also a mandatory requirement during application for TSC registration. It serves to certify a candidate’s medical fitness for contract/ Internship or permanent TSC employment. It is only filled once.

TSC GP 69 Form Structure

The form is divided into two main parts:

Part 1: Completed by the teacher seeking medical report. It includes the teacher’s name and the type of employment sought.
Part 2: Completed by a licensed Medical Officer after conducting a physical examination. The officer certifies whether the candidate is “fit” or “unfit” for the intended service.

Candidates must also provide their personal medical history on page 2 of the form, including details of any past hospitalizations or serious illnesses.
