This is an all important form that serves the following purposes:

  • Legal Representative: The nominated next of kin automatically assumes the role of a legal personal representative in case the teacher is unable to manage their affairs or in the event of their demise. This includes getting all the pension and gratuity payments.
  • Benefits and Compensation: It is crucial for ensuring the smooth transfer of benefits, such as those from the Benevolent Fund (BBF) or death gratuity and Pension.
  • Emergency Contact: The details provided are used by the Commission to contact the appropriate person in case of an emergency.

How to Complete the TSC next of Kin Form

The form requires the following details:
  • Teacher’s Information: Full name (in block letters), TSC number, ID/Passport number, date of first appointment, and permanent address.
  • Next of Kin and Dependants:
    • Name(s), age, relationship (e.g., Husband/Wife/Son/Daughter/Father/Mother), contact information, and address/phone number for up to five next of kin and ten dependants.
  • Declaration and Witnessing:
    • The form must be signed and dated by the teacher.
    • The declaration needs to be witnessed by the Area Chief and the Headteacher (or Deputy Headteacher) of the institution, who must also sign, date, and officially stamp the form. 

TSC next of Kin Form Submission and Updates

Once completed, the form should be submitted to the TSC. It is mandatory to communicate any changes to your records or next-of-kin information to the Commission immediately. Updates can sometimes be made via the TSC Online Portal. But, the hard copy serves as the best option. Simply fill out the form and send it to the TSC headquarters via courier service providers or postal address.
