Issuance of Death Certificate for all current death occuring in Kenya.

If your application requires the change of name or Age then fill in the Death certificate form and then avail your self at the Civil Registration Office with the following documents.

REQUIREMENTS

1. Original ID Card of the Deceased.
2. Burial permit of the Deceased.

PROCEDURE

NB:After completing the application print the application form and the invoice and submit to the registrar for the death certificate processing.The office is located at ACK Garden House behind NSSF Block,along Bishops Road.
